  • Postdated Checks (for Monthly and Amortization Downpayment)
  • Approved Membership Status Verifications Slip (MSVS)
  • Tax Payment Contract
  • Mayor’s Permit / Business Permit
  • Verified Financial Statement (must be from the previous year)
  • SEC or DTI Registration
  • Certificate of Secretary (if applicable)
  • Income Tax Return (ITR) – (Previous 1 years)
Necessary Documentation for Self-Employed Individuals Seeking a PAG-IBIG Loan
  • Up-to-date Income Tax Return (ITR)
  • Service Record – for Government Employee
  • Valid Employment and Compensation Certificate 
  • Last Three Months Pay-slip
  • ESAV with 24 Months Contribution – Copy
  • Photocopy of Passport
  • Updated ESAV with 24 months of contribution – Copy
  • Remittance Proof Contract (for the last three months)
  • Power of Attorney – Verified and Valid
  • Job Contract (must Verified by POEA) and English translation (in case it’s in a foreign language)
  • Income Proof – Photocopy or Original. It must be validated for PAG IBIG loan processing. Meanwhile, it must be verified by the Information Officer in the country where you’re working.
  • Employment and Compensation Certificate (Consularized) – required if no Job Agreement is available

  • Two signature-bearing IDs
  • Contract of Authorized Representative/ Head Officer
  •  SSS/GSIS No.
  • Filled multi-purpose PAG Loan Application
  • Certificate of Loan Application Acceptance
  • Disclosure Contract on Loan Transactions
  • Promissory Note (PN)
  • PAG House Loan Application
  • Specimen Signature

  • Preliminary Loan Counseling Questionnaire
  • Deed of Restriction Contract
  • Contract to Sell
  • Loan & Mortgage Agreement (LMA)
  • Absolute Sales Deeds (H4-75 & H4-74)
  • Certificate of Housing with Lot Acceptance (CHLA)
  • Medical Questionnaire (Applicable just when loaner is up to 60)
  • Agreement of the Sell Contact to Conformity
  • Deed of Assignment (DOA) along with Special Power of Attorney

  • PAG IBIG Loyalty Card
  • Voter’s ID
  • ID of Integrate-Bar of the Philippines (IBP) 
  • Overseas Filipino Worker (OFW) ID
  • Passport – issued by a Foreign Government or Government of the Philippine 
  • Senior Citizen Card
  • Professional Regulatory Commission (PRC) ID
  • Social Security System (SSS) Valid Card
  • Overseas Workers – Worker Welfare Administration’s (OWWA) ID
  • Government Service Insurance System (GSIS) – eCard
  • Government Office & GOCC ID (AFP ID)
  • NSO Copy of your Birth Certificate. Please scan and email this documentation.
  • Seafarer Identifications – Record Book (SIRB)
  • Must obtain the Barangay Clearance – You may transfer its copy or bring the authorized representative with you physically.
  • Must Share your PAG Contribution Printout by scanning and emailing this document
  • Local Employees: COE document’s scanned copy with your two-month scanned salary slip
  • Self-employed/OFWs/Seaman: Scanned Job Contract, Passport, and Seaman’s Book
  • Print 5 back-to-back copies for both PHLA1 and PHLA2
  • You must complete the borrower’s data in a single copy of PHLA1, as our office will type the others
  • Sign all 5 copies of PHLA2 without including the date
  • Print and sign 5 copies of the Special Power of Attorney
  • Return all original documents to the in-person branch or mail. The address is:
